 The Truth About Chronic Hives: When a Rash Isn’t Just a Rash

Anyone can get hives, and in many causes, the rash goes away quickly without treatment. But what if hives don’t go away?

If you have a red, itchy rash that lasts more than six weeks, even if it comes and goes, you have chronic hives. While the exact cause of the rash isn’t always known, you can get relief with the right treatment.

About hives

Hives, or urticaria, are raised red or skin-colored bumps or patches on the skin that usually cause intense itching. The rash occurs when your immune cells release histamine, causing blood vessels to expand and leak fluid, leading to the bumps. Histamine also irritates the sensory nerves, triggering the itchy sensation.

Hives come on fast and may disappear within a day, but they also can last a few days or a few weeks. It’s also not unusual for the rash to disappear in one area and reappear elsewhere.

Though very uncomfortable, hives are generally harmless. However, you should seek urgent care if your hives make it hard to swallow or breathe or cause lightheadedness. These symptoms are signs of a serious allergic reaction and need immediate medical care.

While hives have no cure, taking steps to relieve the itching is important to avoid skin damage and reduce the risk of infection. Simple remedies like cold compresses or over-the-counter (OTC) anti-itch creams can soothe the discomfort.

When hives turn chronic

Hives can last several weeks, but when they stick around for more than six weeks, we consider the condition chronic. For some people, hives may persist for several months.

Causes of chronic hives

Experts say that 80 to 90% of people with chronic hives don’t know the cause. In some cases, the rash is caused by an underlying autoimmune disorder, such as lupus or rheumatoid arthritis.

Despite what you might think, allergies rarely cause chronic hives. Fortunately, even when the cause is unclear, we can provide effective treatments. 

Getting relief from the itch

Antihistamines are the primary treatment for chronic hives. However, the right medication and dose can vary from person to person.  

We may also recommend a topical anti-itch or corticosteroid cream if your case is mild. If your hives are severe, you may benefit from a short course of oral corticosteroids. 

If antihistamines can’t fully control your flare-ups, we may prescribe medications that block the production of histamine or other chemicals involved in the allergic or inflammatory process.
