There are many different health insurance plans, each with rules and regulations specific to the plan.  As your healthcare provider, Associated Skin Care Specialists is obligated to follow those regulations in how we report services provided to you.  With the great complexity of healthcare insurance coverage, we must rely on you to be aware of what your health insurance does and does not cover.

If your insurance plan is not listed above, you should contact your insurance company prior to scheduling an appointment.  Some of the plans listed above may also have restrictions on the types of services that can be provided so we encourage you to check with your insurance plan so you do not incur any unexpected expenses.  If Associated Skin Care Specialists is not contracted with your insurance, you will be responsible for any balance that is considered out of network.  Please present your insurance card at every visit.  Please contact our Business Office (763-502-2951) as soon as possible when you have updated insurance information.

Your portion of the charges are to be paid at the time of service. Be familiar with what your insurance company does and does not cover. You are responsible for paying any deductible amount, co-insurance, or any other balance not paid for by your insurance. To find out more information about your insurance coverage, please call the customer service phone number listed on your insurance card or contact your employer for more information.
